Middleton PCN

Who are we? What are we? and why? 
 
Middleton Integrated Healthcare is one of two arms which make up Middleton Primary Care Network (PCN).
 
A PCN is a group of geographically linked GP practices that work together to support patients in the community. The goal is to provide more personalised, proactive care by enabling GPs to work together at scale, improving patient access to a wider range of health professionals. PCN’s are designed to serve "neighborhood" populations, typically ranging from 30,000 to 50,000. PCNs are funded through the Network Contract Directed Enhanced Service (DES) which provides funding for additional staff and enhanced services. 
 
Your own GP practice is still your main point of contact for appointments, prescriptions, and your usual care.

Rochdale Road Medical Centre
Woodside Medical Centre Middleton
Windermere Road Surgery
The Family Practice
The Junction / Alkrington surgery 
 
There are other surgeries based in Middleton not in the list above, this means they are either the second arm of the PCN or part of another PCN.
 
You may have noticed posts from both your GP surgery and our Primary Care Network (PCN) — so we wanted to explain what that means for you.
 
By working together as a network, practices can:
 
• Share additional services and specialist staff
• Offer more support for things like long-term conditions and wellbeing
• Run community health events and initiatives
 
What does this mean for you?
 
You’ll continue to contact your usual GP practice as normal. The PCN works alongside your surgery to provide extra support and services when needed.
Middleton Primary Care Network (PCN) is a collaboration of GP surgeries across Middleton of which our practice is a member. Working in partnership to improve access to services across the township, the PCN now has a number of professionals to offer increased access to health care, these include:
  • Clinical Pharmacists
  • Pharmacy Technicians
  • Social Prescribing Link Workers
  • Mental Health Nurse
  • Psychologist
  • Paramedic
  • First Contact Physiotherapist
  • GP Assistants
  • Phlebotomists
  • GP's
The Middleton PCN has access to different services including Physiotherapy (MSK FCP), SPLWs, GP appointments, Phlebotomy and Mental Health appointments. The PCN also provides health awareness days as well as the option of self-referring for mental health, housing, financial and relationship problems. Enhanced access appointments may include services like cervical screening, childhood vaccination clinics and health checks.  People will be able to see a member of the general practice team, a GP, nurse or other health professional. They may be with a different clinician than usual.  This clinician will have access to your health records and will be able to treat you in the same way as we would.
